If you are a US citizen, you have to have a valid passport to visit Costa Rica and that passport can not expire 90 days from when your expected leaving date is.

Once entering, Immigration officials will stamp your passport with a 90 tourist visa.

Yes, you can visit without visa. Just fill the blanks on the attached site to read:
"Visa required, except for Alien residents of U.S.A., provided holding residence permit which is valid at least six months beyond the date of arrival." For how long you may stay (30/90 days) depends on your nationality.

I was just there a couple years ago. I needed my passport, but not a Visa. The US has an agreement with most latin american countries to where US citizens don't need Visas, but still need passports.
